Primary care for every family member, services include treatment for colds, flus, infections, high blood pressure, diabetes, wellness visits (immunizations, shots, cancer testing), work, school and sports physicals, DOT and other physicals, women's health (Pap smears, breast exams, STD testing/treatment), pregnancy care and family planning services, minor procedures, weight management, management of diabetes and high blood pressure, child health (pediatrics and adolescent care), geriatric medicine, behavioral health and wellness issues, such as depression and anxiety.

Provides information about services that may be available to individuals with developmental disabilities and their families.

Provides transportation within Granite City Township. Only for Granite City Township residents.

Provides couples counseling services to address many specific issues, including communication problems, sexual difficulties, conflicts about child rearing and/or blended families, substance abuse, financial problems, anger, infidelity, divorce and might also be helpful in cases of domestic conflict.

Health screenings are provided for the following: Cardiovascular, Sexually Transmitted Diseases and HIV, Diabetes, Lead, and tuberculosis. Other screenings available upon request.

Provides general medical care services that focus on prevention and early detection of health problems through physical examinations, blood pressure checks, mammograms and other procedures.

Provides Affordable Care Act certified application counselor services.

Pediatricians and pediatric nurse practitioners provides preventative care as well as the treatment of both acute and chronic conditions in children, services include routine and well child exams, immunizations, school and sports physicals, same-day sick appointments, developmental evaluations, management and treatment of chronic conditions, such as allergy and asthma, ADHD and behavioral issues.

Assists individuals with food stamps/SNAP appeals and/or complaints.

Provides gas assistance to clients. Dollar amount of utility payment assistance under this program is based on eligibility requirements and number of members in household.

Loans made by USDA Rural Development to low and very low income applicants to buy, build or repair homes--payments may be subsidized.

Maintain supplies of blank tax forms and archives of past versions of tax forms for reference purposes.
