MERIL is a community-based, non-residential program, designed to promote independent living and to enhance the quality of life for persons with disabilities and seniors by empowering them to control and direct their lives and thus participate actively and independently in society.

Family Case Management: Provides medical, social, and educational services to those in need.

Case Management: Provides assessment of the clients needs and then initiates and monitors appropriate services as needed.

Provides case management to individuals and families with meetings, setting goals with follow-up progression, employment and/or education opportunities.

Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs.

Family-centered Services are provided to families and children in their own homes when a child abuse or neglect investigation/family assessment response determination is preponderance of evidence, unsubstantiated-preventive services indicated, or services needed. These services are also provided to families who voluntarily seek help. Family-centered Services include a range of treatment and support services and are provided, as appropriate to the case treatment plan.

Provides medical case management services for individuals who are HIV positive.

Family development case management designed for families which may need extra support to achieve their goals.

Provides a case management for caregivers of child who has been a victim of sexual or physical abuse. Services include assistance with investigation, prosecution and treatment. Counseling referrals and information on community resources are also provided.

United for Families (UFF): case management for the whole family to help stabilize and support them on their road to restoring self-sufficiency, increased financial security, prevent homelessness, and prevent school mobility challenges for school-age children.

Family Development and Family Support: On-going case management with one-on-one meetings setting goals with follow-up progression, employment opportunities, etc.

Consumer Support Services: Provides case management services for chronically ill adults who are discharged from a State-operated facility.

Supportive services for homeless individuals and/or at risk of homelessness and/or substance abuse and/or mental health issues. Services include: Case Management, referral services, rental assistance, emergency assistance, transportation and medical assistance.

Offers comprehensive mental health services to children, youth, adults, and their families. Through a wide range of programming and professional staff, Clark Center offers skilled assistance and support to children, teens, and adults experiencing behavioral or emotional health challenges.
