Eligibility
Individuals with a life expectancy of 6 months or less and a decision to focus on comfort care (rather than curative) with pain control and symptom relief.
Required documents
- Client's physician must certify that they meet hospice criteria of having terminal illness with six months left to live
- Medical/Psychiatric records
