Undesignated Temporary Financial Assistance | Missouri Council of the Blind

Eligibility

Individuals who are legally blind in both eyes

Required documents

  • Complete application
  • Photo ID
  • Written verification of legal blindness from an ophthalmologist or other reasonable authority (obtained within the past year), including a description of the applicants eye condition, visual acuity, and field of vision
  • Any supporting documents such as letters, bills, statements or price quotes that will aid the Committee in processing request should accompany application
  • Proof of income
  • A signed release of information form gives the Committee permission to verify applicant information with the provider/business for whom applicant is seeking payment

Fees

  • Members of the Missouri Council of the Blind shall receive the full benefits of the Special Services Program as set forth by the Special Services Committee
  • Those who are not members of the Missouri Council of the Blind shall receive 50% of these benefits offered by the Special Services Program
  • When possible, grants are made payable to the third party, call for details
