Required documents

Proof of income -Social Security cards for all household members -Birth certificates for all household members -Cold month heating bill (December, January, February) -Electric Bill -Proof of home ownership or a landlord agreement if renting (a 5% contribution is required from the landlord if the applicant's income is over 200% of the poverty guidelines)
