- Patient must be in active cancer treatment
- Patient must be able to walk, use wheelchair or crutches to and from the vehicle and get in and out of the vehicle on their own
- Children under age 18 years must be accompanied by an adult

Services include Non-Emergency Medical Transportation, Wigs, Disease/Disability Information, General Health Insurance Information/Counseling, Health Related Temporary Housing, Health/Disability Related Support Groups, Smoking/Vaping Cessation, and Voluntary Health Organizations.
