Eligibility

Must be an unemployed or self-employed worker whose unemployment was caused as a direct result declared disaster Must be a US national or qualified alien Must not qualify for regular unemployment insurance from any state Must have worked or were self-employed in, or were scheduled to begin work or self--employment in one of the counties listed above Must establish that the work or self-employment they can no longer perform was their primary source of income Can no longer work or perform services because of physical damage or destruction to the place of employment OR because of injury caused by disaster OR became the breadwinner of the household because of the death of the head of household OR due to closure of the work facility by the federal government

Required documents

Proof of employment or self-employment or were scheduled to begin work when the disaster occurred. Social Security Number Name and address of last employer or prospective employer Copy of most recent federal income tax form or check stubs (self-employed individuals will need Schedules SE, Schedule C or Schedule F)
