- Child's immunization record - Proof of insurance - Those without insurance, must show proof of residency to be considered for the department's sliding fee program

- Mon-Fri - 8:30am to 10:30am and 1:00pm to 3:00pm - No walk-in immunizations are given during the morning of the first Friday of the month or during the afternoon of the last business day of the month
