Completed Guest Form (Available on Website) - Current Photo ID (Driver's License if applying for Senior Boxes) - Current Address (If applying for Senior Boxes need proof of address such as utility bill, voter registration card, or state ID) - Date of Birth FOR EVERY MEMBER OF THE HOUSEHOLD - Monthly income (will need to know income to determine eligibility for some government programs)

1st Thursday of month: 2:00pm to 6:00pm - 2nd, 3rd, 4th Thursdays: 10:00am to 2:00pm
