Application process

Apply in person at office and complete passport application, which will then be mailed to individual home after verification

Fees

Fees vary for passport because of the type of passport desired and the age of the individual applying, the Department of Public Health charges a $25.00 execution fee for applications and charges a $10.00 fee for obtaining necessary photographs
