-Must not have any working air conditioning in their home AND one of the following: age 60 years and older, physically disabled or critically ill (requires a completed physician form stating they have a critical medical condition)
  • One AC unit per household

  • Medical Statement/Proof of disability or critically ill
  • Social Security Card for all household members
  • Proof of Income for the previous month for all household members
  • Photo ID
