Description

Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs. Services includes case/care management. DRIVING DIRECTIONS From Highway 63 South, take Highway 72 East. After passing Salem Memorial Hospital, go approximately 1/4 mile and turn left onto J Highway.
