Boone County in Missouri
Provide Care Coordination to older persons and their family caregivers, through case/care management including assessment, care plan development, monitoring and assistance in accessing public and private services. Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs.
